USB front panel to 10 pin header

It seems most of the usb front panels have only one USB port (and a card
reader) or 4 USB ports and no connection to the 10 pin header of a
motherboard.

Does anyone know of one that has a minimum of 2 USB ports and connects to
the 10 pin header?  Integrated card reader and firewire would be nice but
not essential.
