This is something that I finally got around to looking into
about 2.5 years ago. ("Finally", because I'd been wondering
about it for many years.) After a few months of searching
in my spare time (interlibrary loan, on-line, e-mails, etc.),
I typed up some preliminary notes and references. Then,
about a year ago, I posted an unedited version of these
notes in response to someone's question.
