Hi - 

Are you using XSLT to transform your notification? If so an error in
the XSLT will result in a failed delivery. 

What delivery channel are you using? SMTP? File? Something you
developed? 

Also check that you've specified the right device in your
subscription. 

Check the Application Event Log (windows event viewer), there will
likely be an event in there related to the failure.
