Help

I have purchased the Upload in Background extension and installed it,  
restarted Dreamweaver,  and when i try to put it in a page it gives an error 
message "Please select input file tag"   Where do I go to create an input file 
tag??
 I tried to create a PHP page,  and also to integrate this into a previously 
built and working ASP page.

 Any help will be appreciated
