Email?

Pretty RAW in HTML & DW CS3.

 I want to design a web page where a user can fill-in their name, email & some 
text and then CLICK the "Submit" and an email is generated to a particular 
email address. I have search GOOGLE & DW Forum and has not found an answer. I 
have spent half day looking & investigating. I believe, I have to setup a 
'form' but need direction in filling up the email fields. I believe few lines 
of HTML code might put me the right direction or somebody can direct me to a 
"How to" page so I can see how it is done.
