I only read what was visible without horizontal scrollling and saw this:

 DateAdded = '#FORM.DateAdded#',
 and this
 Password               = '#Trim(FORM.Password)#',


 That could be a couple of things.
 a reserved problem with password
 or
 trying to set a date to a string
 or 
 something else.
